What HeyForm does

HeyForm is a form builder for small businesses that want forms that feel like a conversation. Its default interactive mode shows one question at a time, as Typeform does, and the same form can instead be shown as a classic page or as a popup on a website. A form can be a survey, a quiz or a contact form.

The field list in the code covers the usual text, number, email, phone, URL, address and country inputs, plus yes/no, multiple choice, picture choice, opinion scale and rating, date, date range and time, file upload and an input table. It also has fields that many open source builders lack: payment, signature and legal terms. Hidden fields and variables carry data from the link or calculate values, conditional logic changes the path through the form, and the end of a form can redirect to a URL. Google reCAPTCHA is available against spam.

Themes control fonts, colors and backgrounds, and custom CSS goes further. Results show drop-off and completion rates, and responses export to CSV.

Integrations

In the open source code, the integration framework holds a single app: webhooks. The README points to Zapier and Make.com, which can receive those webhooks. The hosted service’s pricing page also shows Google Sheets, Airtable, HubSpot, Mailchimp, Salesforce and Slack, but those connectors do not appear in the repository, so if you self-host, plan to connect other tools through webhooks or an automation tool. A JavaScript embed library puts forms on your own pages.

Self-hosting HeyForm

The documented setup is Docker Compose with three containers: the HeyForm app, MongoDB (the Percona Server for MongoDB 4.4 image) and KeyDB, a Redis-compatible store, with images for x86-64 and ARM64. The app listens on port 9157 over plain HTTP, so you need a reverse proxy for HTTPS. One-click templates exist for Zeabur, Sealos, RepoCloud and Alibaba Cloud. Self-hosted instances can use OpenID Connect to sign in through Authelia, Authentik or Keycloak. The documentation gives no hardware figures.

The entire repository is AGPL-3.0, with no proprietary directory. The license page says running it unmodified carries no restrictions; if you modify it, you must publish your changes, and commercial users of a modified version must also credit HeyForm and link to their fork.

Hosted version

The developers run a hosted service with paid plans and a seven-day trial; the pricing page does not show a permanent free plan. Custom domains, branding removal, custom CSS, larger file uploads and team features depend on the plan, and payments collected through forms carry a commission on the entry plan. The README says the hosted service supports development by the two-person team behind HeyForm.

Limitations

Self-hosters get webhooks rather than ready-made connectors. The documented database image pins MongoDB 4.4, so check that version against your own support policy. A small core team means a narrower roadmap than the larger survey platforms, and HeyForm has no participant lists or research exports like LimeSurvey.

Who it suits

HeyForm suits small businesses, freelancers and makers who want Typeform-style lead forms, quizzes and feedback surveys on their own server, including forms that take payments or signatures, and who are comfortable wiring results to other tools through webhooks.
